IMPORTANT INFORMATION FOR ALL VENDORS
CONFLICT OF INTEREST QUESTIONNAIRE
Chapter 176 of the Texas Local Government Code requires that all vendors who have a business relationship with a governmental entity file a Conflict of Interest Questionnaire for Vendor Doing Business with Local Governmental Entity (CIQ) with the Town Secretary. The form is available here. Complete the form and email to email@example.com or mail to Town of Argyle ATTN: Town Secretary/Purchasing, PO Box 609, Argyle, TX 76226. By law, the CIQ questionnaire form must be filed with the Town Secretary for the Town of Argyle not later than the 7th business day after the date the vendor becomes aware of the facts that require the statement to be filed. See Section 176.006(a-1), Local Government Code. A person commits an offense if the person knowingly violates Section 176.006, Texas Local Government Code. An offense under that section is a misdemeanor. NOTE: Vendors are responsible for ascertaining if they need to file the CIQ form. FORM 1295 CERTIFICATE OF INTERESTED PARTIES
The Texas Ethics Commission (TEC) requires Form 1295 Certificate of Interested Parties be completed online prior to entering into a a contract with the Town of Argyle. REQUIRED VENDOR FORM
W-9 REQUEST FOR TAXPAYER IDENTIFICATION NUMBER AND CERTIFICATION
All Vendors are required to submit a completed, signed, and dated W-9 Request for Taxpayer Identification Number and Certification (W-9) to the Finance Department of the Town of Argyle prior to any payments being processed. The
